This is the Go-iire Omamori (sacred robe amulet) of Shin'u Kaijōnin, a Sokushinbutsu (living Buddha) enshrined at Ryūsuiji Dainichibō, the head temple of Yudono-san in Yamagata Prefecture.
I received this amulet when I visited the temple.
It is a famous amulet that has been featured in the media as Japan's Strongest Amulet.
